Caregiver-friendly home planning treats the home as a system: the senior, the caregiver, the daily routines and the equipment have to fit together. We do this as a whole-home assessment that produces a phased plan, so families know what to do first, what to defer and what to budget for.

What the assessment covers

  • Fall-risk audit, room by room.
  • Caregiver workflow — where transfers happen, where the caregiver stands, where equipment is stored.
  • Discharge-from-hospital readiness — what must be in place before a returning patient comes home.
  • Aids and devices — what is in use, what is missing, what is incompatible with the home.
  • Phased modification plan — week 1 priorities, month 1, longer-term.

Working with clinicians and community care

Where the user is already under the care of a hospital, polyclinic, day-care centre or home-care provider, we coordinate so the home matches their care plan. Singapore care services can be at home, in a centre or in a stay-in facility, and the Agency for Integrated Care supports community-based care for seniors living at home.
